To conduct surveys in three (3) target communities to inform Closing the Gap with Social Determinants of Health Accelerator Plan (i.e. New York State cities of Albany, Cohoes, Watervliet). Activities include, but are not limited to:

Conduct community survey to identify social determinants of health (barriers and facilitators) and develop findings
- Design and develop social determinants of health community survey
- Implement social determinants of health community survey
- Develop and submit findings

In conjunction with deliverables, submit regular reports including all program related data (in aggregate form).
