RFP-2016-135 - Development of Two (2) Supported Beds

In an effort to provide additional independent housing opportunities to individuals being discharged from State Psychiatric Centers and/or Article 28 Inpatient Hospitals, are high users of Medicaid services, or are currently residing in a licensed OMH residence (State-operated and /or voluntary Community Residence or apartment who are in need of a residential placement, an RFP is being issued in the amount of $28,647.  This funding will include a onetime only additional two quarters of funds in the amount of $9,549. Additional funding must be utilized for startup costs and operational expenses. Ongoing funding will be $19,098 for two (2) beds.  Providers will be required to have the beds opened and entered into the CAIRS system by February 1, 2017.
