Description

The Contractor shall provide medical supplies, including, but not limited to: Enteral Feeding, Urological, Ostomy, Tracheotomy, Wound Management and Ancillary supplies and formula directly to the residents of the Shaker Place Rehabilitation and Nursing Center (herein after referred to as Shaker Place). Contractor shall provide items as directed by Shaker Place.

The Contractor shall accept assignment and directly bill the resident’s Medicare Part B, Medicaid, secondary insurance, commercial insurance or other responsible party. Contractor shall accept reimbursement(s) received as full payment.
