CONTROLLED INSPECTION SERVICES
- ABOUT THIS RFP:
The Catskill Central School District requests unit cost pricing to perform Controlled Inspection Services for the following Projects as described below:
Project 1: New Middle School Gymnasium Building Construction including Site Work
Project 2: Addition and Alterations to Nurse’s Suite at Catskill Middle School including Site Work.
Project 3: Interior and Exterior Alterations to the Catskill Elementary School.
Project 4: Exterior Pavement Replacement
Project 5: New Parent Drop Off/Roundabout at Catskill Elementary School
Project 6: Interior and Exterior Alterations to the Catskill High School.
Project 7: Interior and Exterior Alterations to the Catskill Middle School.
Project 8: New Field Track and Synthetic Turf at Catskill Middle School.
Special Inspection Agency Requirements
The Special Inspection Agency must be approved and meet all applicable requirements of the NYS Department of State Division of Code Enforcement Administration (Hereafter referred to as NYSDSDCEA). All equipment used must be adequate to perform required tests and all employed personnel must be experienced and educated in conducting, supervising and evaluating tests and/or inspections. Please review Special Inspector Minimum Qualifications prior to submitting a proposal. Proof of qualifications must be submitted to the Owner prior to award.
All inspections must be documented with reports indicating that all work was done in conformance to approved construction documents, and be furnished to the Code Enforcement Official, Owner, and Design Professional. Any discrepancies must be documented appropriately and reported, as well as corrections of it must also be reported to the Code Enforcement Official, Owner, and Design Professional.

SCOPE OF SERVICESThe contract scope includes work related to the District’s Capital Improvement Program, which includes additions and alterations to multiple schools within the District, anticipated to commence in the fall of 2020. Most of the required testing services will be concrete testing, soils testing and compaction, structural steel, and asphalt placement.
Responsibilities of the Multi-Discipline Inspector to include but not limited to:

- Periodic inspections and testing in accordance with the contract drawings, specifications and as required by the Building Code of NYS. All inspection activities are to be supervised by a NY State Licensed Professional Engineer. Simultaneous inspections shall be performed by the same inspector on a given day, i.e. compaction, rebar, and concrete testing, certified in each discipline.
- Footing bottom inspections.
- Compaction/backfilling by a soil technician.
- Rebar reinforcing steel, formwork, and anchor bolt inspection by an ICC or ACI Concrete Construction Specialist.
- Concrete testing (compressive strength, slump, temperature, air content, etc.) by an ACI Field Testing technician- Grade I. Laboratory Concrete testing (compressive strength, etc.) by an ACI Concrete Laboratory Technician.
- Fireproofing inspection:
- Density / adhesion /cohesion /thickness / review of product data sheets
- Masonry inspections by an ACI Masonry Field Technician for field verifications and ACI Masonry
- Laboratory Technician for laboratory tests:
- Material verification / plumbness / proper placement procedures
- Reinforcement verification
- Mortar strength / grout compressive strength

- In-place testing of asphalt thickness, surface smoothness and density. Verify height of traffic-calming devices.
- Structural Steel Inspections by the appropriate ICC certified Inspector (“Steel and Bolting” or “Welding”). Multi- pass filet welds, penetration welds and other welds as called out by the Architect or Engineer of Record shall be Non-Destructive Tested (NDT) by an AWS Certified Welding Inspector:
- Plumbness, alignment and erection.
- High strength bolts/torque.
- Welds, certifications
- Review mill certifications and steel fabrication.

- Special Inspection Notes:
- Written reports are to be provided for each day’s work; signed and sealed by a NY Licensed Professional Engineer.
- Copies of all special inspection reports shall be provided to the Architect and Owner’s Representative/Construction Manager on a weekly basis. Any inspection/ report that indicates a failure or a deviation from the contract documents shall be provided to the Architect and Owner’s representative immediately within (24 hours of the inspection).
- The testing agency shall be required to maintain a special inspection book. The book shall be a three-ring binder which contains copies of all inspection reports. At the completion of the project (3) complete copies of the inspection report book shall be turned over to the Owner.
- All inspection reports shall include the date of the inspection, the specific location where the inspection was conducted and that the work inspected was done in conformance with the construction documents or shall be corrected, along with all other pertinent information.
- Where fabrication of a structural assembly is being performed on the premises of a fabricators shop, special inspections shall be required.
- Grade and mill test reports are required for main stress carrying steel elements.
- A special inspector shall be present when pile foundations are being installed and during tests. The special inspector shall make and submit to the Owner and Architect the official records of the installation of each pile and results of the load tests. Records shall include the cutoff and tip elevation of each pile.
- Prior to the placement of fill, the special inspector shall determine that the site has been prepared in accordance with the contract documents.
- During fill placements and compaction, the special inspector shall determine that the material being used, the maximum lift thickness and the in place dry density comply with the contract documents.
- Special inspections are required for sprayed fire-resistant material being applied to structural elements and decks, including thickness, density, and bond strength.

- The District, the District Architect or District Construction Manager on the District’s behalf, may make and distribute copies of the test results and reports in connection with the proposed project without incurring obligation for additional compensation.
- The District has title to this property and the right of entry for these inspections.
