REQUEST FOR PROPOSAL COUNTY OF CAYUGA 

Sealed proposals, plainly marked as such will be received at: 

Office of the Clerk Cayuga County Legislature County Office Building, 6th Floor 160 Genesee Street Auburn, New York 13021 

on or before 2:30 P.M. on Tuesday, August 12, 2003 at which time proposals will be opened for: 

Project Title: PROVISION OF PHYSICAL AND SPEECH THERAPY SERVICES AT CAYUGA COUNTY NURSING HOME & POTENTIALLY OTHER COUNTY DEPARTMENTS 

Specifications, General Conditions, Standard Contract Clauses for Cayuga County may be obtained at the Office of the Clerk of the Cayuga County Legislature during normal working hours: 9:00 A.M. to 5:00 P.M. Proposals must be submitted net, less all taxes. A signed Affidavit of Non?Collusion must accompany all proposals. The Cayuga County Legislature reserves the right to reject any and all proposals. The County of Cayuga does not accept faxed or e-mailed proposal documents.
