The requirements listed below must be included in bid package for a bidder to be considered responsive:
Submit a sealed bid proposal either mailed or delivered to: Robert Van Vlack, City Purchasing Agent, Office of the Comptroller – 4th Floor, City Hall, 83 Broadway, Newburgh, NY 12550.
All bids must be clearly marked as “#2.25 –“Police 3D Scanner”.
Submittal Form A - BID - 3D Scanner, Accessories, and Software (Signed). All items including delivery. The City of Newburgh, NY is tax exempt.
Submittal Form B – BID – Support (Signed)
Submittal Form C – BID – Training (Signed)
Submittal Form D - BIDDER ACKNOWLEDGEMENT FORM (Signed)
Submittal Form E - NON-COLLUSION BIDDING AFFIDAVIT (Signed and Notarized)
Submittal Form F - CERTIFICATION OF COMPLIANCE WITH THE IRAN DIVESTMENT ACT (Signed and Notarized)
