Summary of Specifications:
The Dutchess County Department of Health (DCDOH) is seeking proposals from qualified vendors to conduct a community survey related to tick-borne disease. The survey includes a maximum of 58 questions with a minimum of 49 questions based on skip patterns. This survey will provide insight on knowledge, attitudes and behaviors and consequently allow DCDOH to develop future work plans and interventions based on the results.
