Description

The Dutchess County Office for the Aging (“DCOFA”) is seeking to contract for Consumer Directed
Home Care Services under Expanded In-Home Services for the Elderly Community Based Services
Program. The awarded contractor(s) (“Contractor”) will provide services in conjunction with
established DCOFA program policy and procedure.
The purpose of this service is to support the consumer’s long-term care needs and preferences as
well as nursing home diversion and Medicaid spend–down prevention through the development of consumer
directed service delivery. Consumer Directed Home Care participants will have greater flexibility
and freedom of choice in obtaining services. Consumers who are deemed eligible will recruit, interview,
hire, train, supervise, schedule, and terminate their employees. The Contractor will function as a
Fiscal Intermediary for the Consumer Directed aide
