Summary of Specifications:
The County, through its Office for the Aging (“OCOFA”), is seeking proposals from qualified Offerors to provide efficient and cost-effective services to eligible County residents age sixty (60) and over through the Community Services for the Elderly Program (“CSE”) as more fully described below. Multiple contracts may be awarded to ensure service coverage throughout the County. CSE was established to provide community-based, supportive services to frail, low-income individuals who are age 60 and over and need assistance to maintain their independence at home. The goals and objectives of CSE are as follows: • Enable the elderly to remain in their homes and participate in family and community life; • Reduce the reliance on institutional care; • Provide cost-effective, non-medical services; • Enable access to necessary information and support; and • Promote the continued involvement of informational caregivers. OCOFA receives CSE funding through a block grant from New York State. Such funding enables OCOFA the flexibility to determine the specific unmet needs of the County’s elderly population and shape the way the delivery system is organized to respond to those needs, within the general parameters of CSE’s intent and goals. The types of services that CSE funding is currently available for are as follows: • Health Promotion and Wellness Activities; • Information and Assistance; and • In-Home Contact and Support (Shopping Assistance, Friendly Visits, Telephone Reassurance)
