Summary of Specifications:
The County, through its Office for the Aging (“OCOFA”), is seeking proposals from qualified Offerors to provide efficient and cost-effective services to eligible County residents age sixty (60) and over through the Expanded In-Home Services for the Elderly Program (“EISEP”) as more fully described below. Multiple contracts may be awarded to ensure service coverage throughout the County. EISEP provides only non-medical in-home services, case management, non-institutional respite and ancillary services to assist eligible individuals, age sixty (60) and over, to help them remain in their home. Services are provided to functionally impaired elderly who are in need of community-based long-term care but who are not eligible for similar services under Medicaid. The overall goal of EISEP is to provide access to, and the availability of, appropriate and cost-effective non-medical support services for non-Medicaid dependent elderly. EISEP is not an entitlement program and, thus, does not serve all older individuals in need. This program operates under a fixed, capped budget and requires client cost-sharing. Although targeted to low income elderly, clients who are financially able may receive needed services, provided they pay for the services. OCOFA receives EISEP funding through a block grant from New York State. Such funding enables OCOFA the flexibility to determine the specific unmet needs of the County’s elderly population and shape the way the delivery system is organized to respond to those needs, within the general parameters of EISEP’s intent and goals. All individuals referred to EISEP are assessed by OCOFA case managers for eligibility. OCOFA case managers then develop an individual care plan for those individuals who are determined to be eligible to receive services under EISEP (“clients”). OCOFA case managers coordinate EISEP in-home care services to support the efforts of a client’s informal caregiver(s). EISEP services are used to supplement, not replace a client’s natural support system.
