The County, through its Office for the Aging (“OCOFA”), is seeking proposals from Offerors to provide certain services to eligible County residents in accordance with Title III-E of the Older Americans Act of 1965, as subsequently amended (the “OAA”). Title III-E of the OAA (“Title III-E”), which was added in the 2000 amendments to the OAA, provides funding to state and local programs to assist eligible caregivers of older adults and children.
The services for which Offerors are to submit proposals in response to this RFP are (a) Information and Assistance; (b) Counseling, Support Groups and Training; (c) Respite Care (which includes In-Home Care Aides, In-Home Contact and Support and Adult Day Services); and (d) Supplemental Services (which includes, but is not limited to, Transportation, Home-Delivered and Congregate Meals, Nutrition Counseling, and Personal Emergency Response). Proposals may be submitted for one service or a combination of services. The services may be performed in outreach locations, institutions and participant residences within the County, as appropriate. Multiple contracts may be awarded to ensure service coverage throughout the County.
