Rockland County Correctional Health Faciity is looking for a supplier to provide one step 16 panel drug test kits/ toxicology cups CDOA-9165EFTK as specified, for a one-year term contract with the option to renew for up to four additional one-year terms. Price must be held firm for the first year. Upon renewal, a Price Adjustment request must be made in writing and include the reason for the request, documentation supporting the request (ie, commodity increases), the current pricing, and the requested revised pricing. The Director of Purchasing may use available indexes (e.g. CPI or PPI) to determine if the requested Price Adjustment is reasonable. Quantites may be ordered monthly as needed. Estimated usage is 100 test kit cups per month, but actual quantities may vary slightly. Prices must be FOB destination prepaid and allowed. Brand Name only. Doctors at Correctional Health Facilty will have final say on if sample is approved for award. Bidder must be an authorized distributor for Manufacturer product. Product brochure must be submitted with bid. Samples must be received within 3 business days of request, at no extra cost to the County. Samples will not be returned unless a return label is provided.
