Ulster County proposes to issue an RFP and contract with a community agency, with a proven track record in the delivery and execution of Human Services programs, to work closely with the Ulster County High Risk Mitigation Team to identify and provide comprehensive guidance and support services for persistent high-risk opioid/substance users in Kingston, New York. Kingston, NY is where the preponderance of opioid overdoses and opioid related fatalities in Ulster County occur. Such guidance and support services include collaborating with the County High Risk Mitigation Team to create a customized safety and recovery plan for every client, to work with designated Certified Recovery Peer Advocates to help support the client, establish linkages to harm reduction and treatment services, and to continually and consistently monitor and document every client’s progress. The overarching goal is to make sure that clients do not get caught up in an endless and vicious cycle of recurring overdoses, and that clients have the tools, guidance and support services necessary to help them move through the entire continuum of harm reduction, care, treatment, and a safe and sustainable recovery.
