NOTICE IS HEREBY GIVEN
that the County of Ontario will receive Proposals for Homeless Case Management (R24047). Such proposals must be filed with the Ontario County Purchasing Director, 20 Ontario Street, Canandaigua, New York 14424, on or before 4:00 PM prevailing time April 29, 2024.
All questions pertaining to this RFP shall be forwarded in writing to the Director of Purchasing at the address shown above.
Ontario County reserves the right to reject any or all proposals.

Statement of Work
The work involves two (2) staff providing a moderate level of case management activities and services to the homeless population, who require more ongoing support in order to function as independently as possible in the community. Each contractor staff will work with 20 – 30 clients and will have frequent client contacts, a minimum of two per month with each. Work is performed in the office and in the field, and may require transporting a client to relevant appointments or activities. The work is performed under the supervision of a Principal Social Welfare Examiner (Canandaigua office) or a Head Social Welfare Examiner (Geneva office). Contract staff must have reasonable access to transportation to complete field work.
