SCOPE OF SERVICES
This Request for Information (RFI) is for the purpose of alcohol/ drug testing, screening services and employee physicals for the Rochester City School District (RCSD). All work shall be completed according to the enclosed specifications.
Vendor shall provide alcohol/ drug testing and screening services; employee physicals as required by the Rochester City School District. Alcohol/ Drug testing services to consist of monthly random testing, on-call testing and pre-hire testing. Employee physicals to consist of pre-hire, yearly as well as return to work physicals. Vendor shall perform work in conformance with all Federal, New York State, City of Rochester, RCSD Laws and Regulations by certified Technicians and a certified Medical Review Officer.
The Employee Benefits Department will require the consultant to provide pre-employment drug testing (5-10 panel) for district employees and random drug and breath alcohol testing as needed during normal work hours or after hours as needed. Other services may include hair follicle tests and hepatitis B immunizations. A report must be sent to the District within 24-48 hours after testing. Sometime fitness for duty physicals may be required. Tests that may be required for staff working at Strong Hospital include but are not limited to: Physical Exam, Tuberculin Skin Test(PPD), Chest X-ray, Rubeolla, Rubella, Tetnus/Diphtheria, Hepatitis B Vaccine Series and Surface Antibody Titer and Varicella Titer.
