RYN-2425-010 - Related Services: Occupational Therapy, Physical Therapy, Speech/Lang. Therapy

The Rye Neck Union Free School District is seeking a related services provider that is able to deliver related services including Occupational Therapy, Physical Therapy, Speech/Language Therapy for grades K-12 at a competitive cost with the following conditions:
 

• Provider must be able to provide related services in Occupational Therapy, Physical Therapy, Speech/Language Therapy
• Services should be available at the students' homes, which will be located in Mamaroneck, NY and Rye, NY
• Services will be aligned with parameters in students' IEPs
• The student profile may include: limited verbal to full verbal communication, limited physical mobility to mobility independent, and need for ADL skills to be self sufficient
• The intent is long-term service, not recuperative
• Services may be provided during all 12 months of the year (school year and extended school year, i.e. summer months)
• Agencies will need to be able to develop the service curriculum/plan themselves
• All service providers must be NYSED/ NYS licensed/board certified Occupational Therapy, Physical Therapy, Speech/Language Therapy 
• Progress Reports, Assessments, or Evaluations of students will be required no less than on a quarterly basis.
