Interested consultants should submit a proposal that includes:
 

1. Background Information Relating to Proposer

• Proposer’s legal name and any trade name/”doing business as”
• Address of principal office and location that will provide the services
• Year Proposer was established
• Total number of employees
• Number of employees proposed to be dedicated to this project
• Proposer’s experience with providing the above services
• Identify employees/staff proposed to be dedicated to this project with their relevant experience

2. Executive Summary
   • Overview of the approach and methodology to be used in the assessment.
3. Qualifications and Experience
   • Description of relevant experience, particularly with community colleges or similar institutions.
   • Resumes of key personnel who will be involved in the project and their relevant experience in providing the requested services.
   •    Provide a brief history of the firm including its size, location of office(s) and names of Principal(s).
4. Project Approach and Work Plan
   • Detailed description of the proposed approach to the assessment, including methodologies for data collection and analysis.
   • Proposed timeline for completion of each phase of the project.

5. Budget and Fee Structure

•    Detailed cost proposal, including consulting fees, travel expenses, and any other associated costs. Failure to include the fee proposal amount with the proposal may disqualify the proposal. If the fee is a not-to-exceed amount based on your estimate of hourly rates and costs necessary to complete the scope of work, you must provide a detailed budget outlining all such hourly rates and cost estimates.

6. References
   • Indicate three (3) current or recent references for which you provided similar services, include:
   • Client’s name
   • Contact information for client, including current address, contact name, title, telephone number and email address.
   • Description of Services provided to each of the clients.
7. Timetable

•    Provide a timetable for completion of the assignment (if the College has set forth specific deadlines above, then provide a timeline for meeting those deadlines, including all interim milestones necessary to complete the project).
