The contractor shall agree to provide all of the requirements listed within the Statement of Work (SOW). All orders placed against the BPA are subject to the terms and conditions within the agreement.  Extent of obligation. The Government is obligated only to the extent of authorized purchases/orders actually made under the BPA. Participants must provide a signed consent form and liability waiver. The Contractor shall agree to provide all the requirements listed within the statement of work.  Background: From the U.S. Department of Health and Human Services: Being physically active is one of the most important actions that people of all ages can take to improve their health. The evidence is clear physical activity fosters normal growth and development and can make people feel better, function better, sleep better, and reduce the risk of a large number of chronic diseases. Health benefits start immediately after exercising, and even short episodes of physical activity are beneficial. Even better, research shows that just about everyone gains benefits: men and women of all races and ethnicities, young children to older adults, women who are pregnant or postpartum (first year after delivery), people living with a chronic condition or a disability, and people who want to reduce their risk of chronic disease. The evidence about the health benefits of regular physical activity is well established, and research continues to provide insight into what works to get people moving, both at the individual and community level. Achieving the benefits of physical activity depends on our personal efforts to increase activity in ourselves, family, friends, Veterans, and colleagues. Action is also required at the school, workplace, facility, and community levels. Today, about half of all American adults 117 million people have one or more preventable chronic diseases. Seven of the ten most common chronic diseases are favorably influenced by regular physical activity. Yet nearly 80 percent of adults are not meeting the key guidelines for both aerobic and muscle-strengthening activity, while only about half meet the key guidelines for aerobic physical activity. The lack of physical activity is linked to approximately $117 billion in annual health care costs and about 10 percent of premature mortality.   Performance Period: The performance period will be on the fiscal year calendar (October 1 September 30). Contractor will provide a fitness instructor to instruct classes to Veterans five days a week (nine hours total each week). Contractor will provide a personal trainer to deliver training to Veterans two days a week (four hours total each week). Classes will be held in the Butler VA Health Care System s Wellness Center and at the contractor s facility as determined by the point of contact and contractor (except for on Federal holidays when classes will be cancelled). Federal Holidays are as follows: New Year s Day, January 1st or the Friday preceding or Monday following as designated. Martin Luther King s Birthday, 3rd Monday in January President s Day, 3rd Monday in February Memorial Day, Last Monday in May Juneteenth, June 19 Independence Day, July 4th or the Friday preceding or Monday following Labor Day, 1st Monday in September Columbus Day, 2nd Monday in October Veteran s Day, November 11th or the Friday preceding or Monday following as designated. Thanksgiving Day, 4th Thursday in November Christmas Day, December 25th or the Friday preceding or Monday following as designated. Modified hours for Christmas Eve and New Year s Eve shall be coordinated with the VA representative.  Type of Contract: This contract will be under a firm-fixed-price Blanket Purchase Agreement. The contractor will be paid an hourly rate for each hour-long class or personal training hours. Cancelled Classes: The contractor will provide at least 24 hours notice to cancel a class. The contractor will notify the Program Manager or designee immediately if emergency conditions such as severe weather, hazardous road conditions, or personal emergencies prevent the instructor from showing to a scheduled class. For situations beyond the contractor s control, such as facility maintenance of the Wellness Center, the contractor will only be paid for a scheduled class when less than 24 hours notice has been provided to the contractor. The contractor will be paid for scheduled classes when the instructor is present, even if no participants show up. In this case, the instructor must wait at least 15 minutes, and then may leave upon signing a dated, blank sign-in sheet indicating no participants and notifying the program manager or designee. Emergencies: The instructor will call facility medical response staff for any participant injuries or accidents during a class or personal training session. If necessary, the instructor will call 911 for medical assistance. The instructor is responsible to administer care as deemed appropriate (CPR, etc.) for the circumstance until medical care arrives. VA Security Policies: Instructors are required to complete Privacy and HIPAA training and Privacy and Information Security training. The C&A requirements do not apply. A Security Accreditation Package is not required. A Local Business Associate Agreement between Butler VA Health Care System the contractor is not required. The contractor must obtain an identification card from the police department.  Contractor must complete VA Privacy and Information Security Awareness and Rules of Behavior and Privacy and HIPAA Focused Training courses prior to the performance of the contract and annually thereafter. Training must be completed in VA s TMS system (https://www.tms.va.gov/SecureAuth35/). Contractors must use the TMS Managed Self Enrollment method to complete the training in TMS. The point of contact must ensure that all contractors are validated in the PIH domain. Proof of training completion must be verified and tracked by the point of contact. A local business associate agreement is required.  Minimum Contractor Requirements for Service: Contractor must own or lease a facility within 5 miles of the Butler VA Healthcare System which offers a wellness center which provides the following: Pool/Swim Exercise, Group Exercise, Knockout Parkinson s classes, Pickleball classes, TRX (Total Resistance Exercise) Classes, and Yoga. The Contractor shall provide a list of all classes and amenities in which they provide. The Contractor shall be able to provide classes at both their facility and the Butler VA Healthcare System. The hourly fee covers all costs. No membership is needed at the contractor s facility. The instructor must have current cardiopulmonary resuscitation (CPR) certification.  Certification must be on file prior to instructing any classes. Instructor will provide TB test results prior to starting work.   Instructor will have at least one year of experience in fitness class instruction and personal training (certification preferred). Instructor will provide notice of any class changes a week before the class. A weekly schedule is made available and the instructor will ensure all participants sign-in prior to each class. Instructor will ensure all participants have completed and signed a consent form and liability waiver before participating in their first class.
