The purpose of this notice is to identify potential sources and suitable locations and is not intended to pre-qualify or disqualify any potential offers. The Government will not pay for any costs incurred because of this notice. The Government cannot provide any warranty, expressed or implied, as to the accuracy, reliability, or completeness of contents of the furnished information found within this notice. Respondents are advised that the Government assumes no responsibility to award a lease based upon responses to this notice. The issuing of this notice is to conduct market research for sufficient 15,540 Net Usable Square Footage (NUSF)/ 20,979 Rentable Square Feet (RSF) of real property in the delineated area of Warren County, Pennsylvania for use as a Community Based Outpatient Clinic. The space shall include a minimum of 75 on-site parking spaces. The anticipated lease will be a full service lease, to mean all utilities, maintenance, and janitorial services are included for a term up to 20 years (firm and soft). The leased space shall be on the first floor in a contiguous space that meets the Patient Aligned Care Team (PACT) model. Heading east on Hatch Run Road to the intersection of Hatch Run Road and Conewango Avenue. Heading south on Conewango Avenue to the intersection of Conewango Avenue and Jackson Avenue. East side boundary Area in and abounding heading east on Jackson Avenue to the intersection of Jackson Avenue and Irvinedale Road. Heading south on Irvinedale Road to the intersection of Irvinedale Road and Cobham Park Road. The continuing south on Cobham Park Road, turning into Park Avenue and continuing south on Park Avenue to the intersection of Park Avenue and U.S. Route 6 Business/ Pennsylvania Avenue. At the intersection of U.S. Route 6 Business/ Pennsylvania Avenue and Park Avenue, the south on U.S. Route 6 Business/ Pennsylvania Avenue East to the intersection of U.S. Route 6 Business/ Pennsylvania Avenue East and Crescent Park Extension. West side boundary Area in and abounding and heading west on Crescent Park Extension to the intersection of Crescent Park Extension and Ludlow Street. Continuing north on Ludlow Street to the intersection of Ludlow Street and Pennsylvania Avenue West, continuing east on Pennsylvania Avenue West to the intersection of Laurel Street. Heading north on Laurel Street to the intersection of Laurel Street and West 5th Avenue. Then heading east on West 5th Avenue to the intersection of West 5th Avenue and U.S. Route 62/ Market Steet. Only SDVOSB and VOSB firms are requested to submit a Capabilities Statement to VA for review. A summary describing up to three (3) projects of similar size and scope completed in the past five (5) years that demonstrate your experience designing, constructing, and managing Federal leased facilities or health care facilities relevant to the project described above. (3-page limit) Evidence of capability to obtain financing for a project of this size. If desired, the company may also submit a narrative describing its capability. We encourage all businesses and other interested parties to respond for market research purposes. The Government is limited by law (40 USC 278a, as Amended 10-1-81) to pay no more than the appraised fair rental value for space.
