Specifications include, but are not limited to: THERAPY SERVICES The provider shall: Provide Speech, Occupational or Physical Therapy services to persons within Cuyahoga County. Provide preventive education services such as fall prevention and balance promotion exercises as requested. Demonstrate therapy activities to patients’ caregivers during treatment appointments. NURSING SERVICES The provider shall: Provide short-term registered nurse (R.N.) and/or licensed practical nurse (L.P.N.) services to persons within Cuyahoga County as requested. Provide services in accordance with Medicare/Medicaid regulations and the Ohio Department of Jobs and Family Services (ODJFS) Policy and Procedure Manual. Document and provide for each patient the initial assessment, the proposed plan of care and accompanying clinical notes within 7 days of the initial assessment. Document clinical notes for each visit reflecting care given, teaching and observations made. Report patient outcomes, progress, and changes to the care plan to DSAS weekly. BOTH SERVICES One unit of service is equal to one hour The Provider shall also: Accept DSAS referrals for service within one business day. Screen all patients referred for services. Participate in an orientation session conducted by the Division of Senior and Adult Services. Assist the physician in evaluating patients and developing a plan of treatment by performing diagnostic and prognostic tests as appropriate. Give patient care services that the provider is qualified to render in accordance with the plan of treatment. Observe, record and report to patient’s physician and appropriate personnel of DSAS the patient’s response to treatment and any change in the patient’s condition. Dress in a professional manner that allows flexibility in the workplace. For example, scrubs and tennis shoes are the expected forms of dress. Be available Monday through Friday from 7:30 a.m. to 5:30 p.m. Offer flexible appointment scheduling including evenings and weekends.
