Project 1: Choice Program Budget: $520,000 maximum for two (2) years. Minimum of 2.0 FTE Peer Support Specialists. Target Population: Adults (eighteen (18) years of age and older) receiving services from Clackamas County’s Choice Model Care Coordination Team. Service Components: • Provide one-on-one and/or group peer support to individuals being served by the Choice Team, including but not limited to adults who have been recently civilly committed. • Provide information about peer services and outreach to Clackamas County individuals either enrolled in Choice Model services or those that have been recently civilly committed who are in acute care settings, crisis respite, Oregon State Hospital, or jail. • Assist individuals in understanding and navigating their care plans, including making informed decisions about their health and wellness. • Support individuals in identifying and achieving personal goals related to their care and recovery. • Collaborate with the care coordination team to provide peer perspective insights during team meetings and care conferences. • Empower clients to advocate for themselves and make informed choices about their care and wellbeing. • Assist with navigation of treatment and support systems, if requested by individual receiving support. • Build and maintain relationship with community organizations, support groups, and other resources that can benefit individuals being served. • Connect individuals to community-based services, supports and resources. • Use a whole health approach, not only addressing issues of addiction and mental health, but spiritual and physical health, as requested by the individual.
