procure a qualified provider to deliver the mobile behavioral health service component of this coordinated care model. The mobile behavioral health service component should encompass street psychiatry, Medication-Assisted Treatment (MAT), Case Management, Care Coordination, Mobile-Based Assessments and Substance Use Disorder (SUD) interventions utilizing ASAM criteria with peer support incorporated throughout. Eligible applicants must include a detailed description for how physical health and outreach services will be provided in their proposal. The contract for services under this RFP will not include funds for the physical or outreach/relational component. Eligible applicants must provide a sustainable plan to provide these services through formal partnerships, MOUs or through their own organization. This will be an 18-month pilot project primarily utilizing Behavioral Health Choices reinvestment funds and will be used to explore and fine tune the service delivery and financial models for continued sustainability.
