Specifications include, but are not limited to: This Scope is intended to incorporate similar services previously contracted into one RFP. The Scope contains two components to provide education and support for reunification or prevention services. Provider Services can range from low risk education services up to and including high risk of placement prevention. The Contracted Service will deliver a wide range of goal-directed services and education to the family. One component is case management services. Contracted Service will aid the family in meeting the medical, educational, and developmental needs of the children. Contracted Service will assist clients in initiating and attending services recommended by the Agency. Contracted Service will assist with transportation to ensure clients are attending all necessary appointments. Contracted Service will also ensure clients gain the necessary independent skills to continue meeting these needs on an ongoing basis. Contracted Service will educate the family on vocational and/or homemaking skills. Skills include but are not limited to financial budgeting, cleanliness of the home, organization and planning, nutrition, safety and health concerns in the home. Contracted Service will ensure family is linked to county assistance and community supports. Contracted Service will utilize one of the below evidence based programs listed in the “Program Description” in order to provide reunification and/or prevention services to remediate the safety threats or risks, which warrant Agency involvement or referral.
