Specifications include, but are not limited to: • Family Engagement (FE) workers will conduct Family Finding, Crisis & Rapid Family Meetings and Family Group Decision Making (FGDM) meetings and attend Family Team Conferences. • FE workers complete a pre-screening form with the identified family. • FE workers attend agency placement meetings and provide Crisis and Rapid Responses to cases as needed. • FE workers attend shelter care hearings and subsequent hearings as needed. • FE workers conduct initial Family Finding activities with the family and assist the assigned caseworker with ongoing Family Finding activities. • FE workers will be trained in and facilitate FGDM Conferences; preparation for a FGDM conference includes contacting and developing a relationship with the family to identify who should attend, contacting and preparing all invitees, identifying and securing a location for the conference, identifying the date and time of the conference, coordination of the preparation or arrangement of a meal for the conference, addressing transportation and lodging issues (if necessary) for the participants. • FE workers will be trained to assist the facilitator of Family Team Conferences; preparation for a virtual Family Team Conference includes: contacting and developing a relationship with the family to identify who should attend, contacting and preparing all invitees, sending out the invitation (including the MH, D&A, Outreach and CCBHO representatives), completing the needs section and, if services in place, the services section of the “Charting the Life Course” to present to the facilitator prior to the Conference . • Educate and recruit family members from the FTC’s to participate in FGDM. • Attend the Statewide FGDM Conferences as needed. • Attend the Eastern Regional Network meetings for FGDM as needed. • Gather data for Lackawanna County Quality Assurance Department. Data includes date of referral, MCI number, DOB for children, dates sessions were held, and current status (completed, still participating, no longer participating/discharged). • Participate in Cross Systems Transitional Conferences and/or CASSP meetings as needed. • Attends planning meetings and the larger board meetings monthly for the Center for Family Engagement/Systems of Care as needed. • Provide consultation and education services to the community; making presentations to the community related to services provided by the children and youth, mental health/mental retardation, and other human services agencies regarding agency engagement practices (e.g., the Interagency Council) as needed.
