The Department requires at least the services listed below, including the specific tasks and work activities described. Provider’s proposed scope of work should state in detail how it will carry out each task, including the personnel/job titles for completing the task. For each service specified, the Provider should propose criteria to determine when the tasks comprising the service are satisfactorily completed. Provider may propose additional or revised tasks and activities but should explain why each is necessary to achieve the project objectives. • Treatment provider for initial facility must be license eligible for ASAM 3.1, clinically managed low intensity residential services and comply with City and State regulations regarding 3.1 ASAM level of care. • Treatment provider of eligible health care services must be able to bill appropriate payer/be enrolled in PROMISE • Provider must maintain appropriate licensure for the duration of funding from the City. • Provider is required to have a policy that ensures that residents are informed of rules, requirements, and processes. • Provider is required to have policies that promote recovery by requiring resident participation in physical, behavioral, and substance use treatment, self-help groups, and/or other recovery supports. • Provider is required to have policies and procedures for management of funds received and expended in accordance with standard accounting practices, including funds received from or managed on behalf of residents. • Provider is required to have policies regarding criminal background checks for operators and employees...
