Specifications include, but are not limited to: 4.1.1.1 Medical Assessment 4.1.1.1.1 An in-person medical assessment, including a medical history and physical examination must be performed by the Attending Physician and recorded in the resident’s medical record no later than seven (7) calendar days after admission, at least once annually, or more frequently based on the resident’s condition. The in-person assessment must include the review of the residents’ total program of care, including medications and treatments. The successful vendor will have access to the secured WVVH software to access progress notes for each resident. The writing, signing and dating of all orders may be accomplished via email or facsimile 4.1.1.2 Comprehensive Assessment and Comprehensive Care Plan 4.1.1.2.1 While the comprehensive assessment will be initiated by the WVVH staff, the assessment must be coordinated with the participation of the Attending Physician or their qualified collaborating designee no later than fourteen (14) calendar days after admission, annually, and as required by a change in the resident’s condition.
