Specifications include, but are not limited to: The City of Milwaukee (City) has issued this Request for Proposal (RFP) with the intent to enter into a Citywide Vendor Service Contract with a professional contractor whose expertise includes conducting basic and comprehensive background check services. The services will be provided to various City departments for pre-employment, contractor screening, and contractor employee screening purposes via electronic system. A. The Basic Background Check I shall include: • Primary Name, including any other names used and/or searched. • Seven (7) years address history. • Social Security Number Check (SSN Trace). • State of Wisconsin Criminal History obtained from the Wisconsin Circuit Court System and the Milwaukee Municipal Court system. • Unlimited County Criminal Record Search for all counties lived in for the past seven (7) years, including but not limited to: o Milwaukee County, Wisconsin, felony & misdemeanor records. o Waukesha County, Wisconsin, felony & misdemeanor records. • Unlimited National Criminal Database and fifty (50) State sex offender search. B. The Basic Background Check II shall include: • Primary Name, including any other names used and/or searched. • Seven (7) years address history. • Social Security Number Check (SSN Trace). • State of Wisconsin Criminal History obtained from the Wisconsin Circuit Court System and the Milwaukee Municipal Court system. • State of Wisconsin Driving Record and/or driving record from the states of residence for the past seven (7) years. • Unlimited County Criminal Record Search for all counties lived in for the past seven (7) years, including but not limited to: o Milwaukee County, Wisconsin, felony & misdemeanor records. o Waukesha County, Wisconsin, felony & misdemeanor records. • Unlimited National Criminal Database and fifty (50) State sex offender search.
