The email must include: • Confirmation of acceptance of the terms of the funding outlined in this memo. • Confirmation that the tribal nation/county is accepting funding for activities during the period of March 15, 2023, through March 14, 2024. • Confirmation that the tribal nation/county accepts the full amount of the award the tribal nation/county is eligible to receive as detailed in Table 1. Agencies may accept a lower amount if it is anticipated that the full award will not be used. • The name, title, and contact information of the person authorized to commit the tribal nation/county to the terms of the contract. o A completed DCTS Annual Grant/Contract Application: Condensed, F-21276C and a completed DCTS Summary Line Item Budget: Condensed, F-01601C. o Complete all fields on the forms. o Use “SAPTBG CRRSAA Extended Supplemental Award” as the grant/contract title. o The DCTS Annual Grant/Contract Application: Condensed, F-21276C, must be received in Word document format. o The DCTS Summary Line Item Budget: Condensed, F-01601C, must be received in Excel document format.
