Provide congregate residential sober living beds in designated ZIP codes of Milwaukee. Provide critical transitional housing and supportive services to allow addicts and alcoholics to bridge in-patient treatment and transition to a stable recovery and sobriety Proposers responding to this RFP will provide the following: a statement of qualifications; description of the program, day-to-day staffing plan and overview of standard operating procedures, a listing of support services, a timeline for implementing the program; an evaluation plan; and budget. Metrics: The selected proposer must demonstrate monthly data showing Total number of people served Number of people served beyond previous capacity under this contract (funded beds) Employment status for the residents Percentage of residents seeing progress toward restored family, interpersonal and community connections (by means of a survey or self-reported demonstrated on a spreadsheet).
