Specifications include, but are not limited to: The Division is requesting housing case management for families receiving either an SRAP or FUP voucher. For SRAP voucher families, services will be provided until they complete the voucher (maximum of 2 years) or are terminated from rental assistance. For FUP voucher families, services will be provided until they are accepted into the Moving to Work Program (approximately 6 months). The successful bidders will work with the DFS Treatment Program Manager to identify families who are eligible for one of these vouchers. This involves reviewing the case, talking to the DFS Case worker, interviewing the applicant, and assisting the applicant in gathering required documentation to determine eligibility. These documents include, but are not limited to, DE Driver’s License or Identification Card, Birth Certificates, Social Security Cards, income verification, and any other documents required by Delaware State Housing Authority. Once eligibility is determined the successful bidders will complete the electronic Delaware State Housing Application through their portal. (Access to the portal will be established by DSHA and DFS Treatment Program Manager). The Housing Case Manager will work closely with the family to find, obtain, and maintain housing. Once housed the successful bidder will develop a plan to assess the strengths and needs of the family and plan with the family on obtaining, maintaining, and transitioning into independent housing. The successful bidders will develop a plan to assist families in obtaining housing. This may include, but not limited to: • participating in any community or Department workgroups related to housing and homelessness • networking with landlords and property management agencies across the state to provide knowledge about the rental assistance program • partnering with landlords and property managers to rent to families receiving rental assistance, • participating in training related to homelessness
