Specifications include, but are not limited to: The Contractor will facilitate and oversee visits by traveling phlebotomists to WLS participants’ homes for the purpose of collecting blood samples. The Contractor will work closely with the University of Wisconsin Survey Center (UWSC) to coordinate these visits. UWSC will manage the process, providing WLS participants with information about the study, obtaining informed consent, and scheduling phlebotomy appointments. The Contractor will send a trained and licensed phlebotomist to the participants home on the scheduled interview date and time. They will collect the blood sample, process, store and ship the sample back to ADRC as outlined in Exhibit 1 - Blood Collection and Processing Grid, and in adherence with all the procedures and protocols as delineated in the Contract and any subsequent Statement(s) of Work.
