sterileSTATEMENT OF WORK FOR Leasing/Maintenance of Sterile Processing Transport Trucks DESCRIPTION OF SERVICES. The contractor shall provide vehicle leasing services for 2 sterile processing vehicles plus backup vehicles needed for the Milwaukee VA Medical center. The contractor is responsible for providing all vehicles, equipment, labor, parts, tools, maintenance and repair of all contractor furnished equipment for the lease of two special purpose vehicles (plus backup vehicle(s) as necessary) to be operated by government employees at the Milwaukee VAMC. The contractor is also responsible to ensure that the backup vehicle(s), if needed, are fully equipped, serviceable, and have the same capabilities and features as the 2 primary vehicles. The contractor is responsible for performing to the standards in the contract as well as all local, state, and federal regulations. GENERAL PROCEDURES. The contractor shall provide all vehicles, equipment, labor, parts, tools, maintenance, repair and other related services necessary to provide two sterile processing transport trucks plus backup vehicle(s) if needed for the VA Medical Center in Milwaukee. The vehicles will be used to transport sterile instruments and instruments to be sterilized between buildings located on the Milwaukee VAMC campus. On an infrequent basis, the vehicles may be used to deliver sterile supplies to other VA locations within daily driving distance of the Milwaukee VAMC. The location of service for the vehicles will be: Clement J Zablocki VA Medical Center 5000 W National Avenue Milwaukee, WI 53295 The contractor shall provide the COR at the VA Medical center with a 24/7/365 single point of contact phone number in the event of vehicle breakdown or maintenance needs for unscheduled maintenance. This single point of contact phone number is required to ensure that the medical center has two fully functional vehicles at all times. This single point of contact shall take calls from VA personnel and provide all necessary communication, coordination and scheduling within their company to coordinate unscheduled maintenance and/or replacement vehicles needed for the Milwaukee VAMC. The contractor shall provide a replacement or backup vehicle within 2 hours of notification of primary vehicle breakdown. At no point will VA personnel be making phone calls to other vendors, other repair facilities or other internal contractor entities. Single point of contact must be available to take calls regarding vehicle breakdowns 24/7/365. The COR and Alternate COR at the Milwaukee VAMC shall be notified via email not less than 24 hours in advance of scheduled maintenance needs for the two leased vehicles. If scheduled maintenance will likely result in the VAMC being without a vehicle for more than two hours, the contractor shall provide the backup vehicle when picking up the vehicle needing scheduled maintenance. VEHICLES The requirements for (2) Primary trucks as well as any backup trucks provided are: 18 22-foot diesel engine box trucks (cargo trucks) Temperature control. Working digital readout, in-cab temperature controls to keep the freight compartment between 65 and 72 degrees In cab humidity monitor installed. The monitor shall provide readout of the humidity level inside the cargo compartment of the truck Ratcheting E track Equipment- The trucks shall have 3 rows of e tracks installed on each side of the cargo box, from front to back at 24, 40 and 56 inches from the floor. The e track equipment is used to secure the sterile cargo to the cargo compartment. Truck exterior with no logos or designs Truck shall be white in color Cargo Box interior must be made of nonporous cleanable and wipeable material Cargo box floor shall be metal, and shall be cleanable and wipeable Ability to transport a minimum of 7000 pounds at one time Automatic transmission Cargo box must be lighted Cargo box shall be 8 feet wide (96 ) Trucks load height shall be 48 inches from ground to floor of truck bed. Trucks cargo box shall be a minimum of 96 inches and a maximum of 102 inches (from floor of truck bed to top of the cargo box Both the interior and exterior of all trucks used under this contract must be clean and free of rust and damage QUALITY ASSURANCE. The Government will periodically evaluate the Contractor s performance to ensure services are received through intermittent on-site inspections and receipt of complaints from facility personnel HOURS OF OPERATION. The normal hours of operation for scheduled maintenance services provided under this contract are Monday - Friday from 8:30AM to 3:00PM, excluding holidays. In the event of unscheduled maintenance or vehicle breakdowns services to provide maintenance, repairs and backup vehicles shall be available 24/7/365. The Federal Holidays Observed by the VA are as follows: New Year s Day January 1st Martin Luther King s Birthday Third Monday in January President s Day Third Monday in February Memorial Day Last Monday in May Juneteenth Day June 19th Independence Day July 4th Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day November 11th Thanksgiving Day 4th Thursday in November Christmas Day December 25th When a holiday falls on a Sunday, the following Monday shall be observed as a legal holiday by U.S. Government agencies. When a holiday falls on a Saturday, the preceding Friday shall be observed as a legal holiday. CONTRACTOR PERSONNEL. The Contractor shall provide qualified employees. The Contractor shall be required to comply with all security requirements of the VA. The Contractor is responsible for ensuring all employees possess all required licenses for operating all vehicles and equipment used in the execution of this contract. The Contractor shall be responsible to ensure Contractor employees providing work under this contract are fully trained and completely competent to perform the required work. Contractor personnel shall wear a uniform with the Contractor's name printed in a neat and professional manner, to be easily visible and recognizable.
