DESCRIPTION OF SERVICES.
The Contractor shall provide all management, tools, supplies, equipment, transportation, lifts, parts and labor necessary to inspect, perform maintenance, and certify 11 Mobile Elevating Work Platforms (MEWP) annually at 19 Civil Engineering Squadron, (Bldgs, 532, 538, 540 and 543) Little Rock Air Force Base, Arkansas 72099 in a manner that will ensure continuous and safe operation. Mobile lifts maintenance, inspection, and certification shall be completed annually between 1 July thru 31 October. Mobile lifts must be certified annually and not allowed to expire unless extenuating circumstances coordinated/approved through mission owner. The applicable MEWPs will be listed in Appendix A. If any additional repairs are needed to certify Mobile Elevating Work Platforms (MEWP), the contractor will provide a repair estimate to mission owner for approval. All work will be in accordance to all Federal and State Law to include all DOD and Air Force Directives.
2 This annual inspection contract signifies a service provider (Contractor) takes total responsibility for the equipment identified in the contract. Except those services covered in paragraph 2 below, the fixed price includes all inspections and applicable certifications.
3 The contractor shall inspect and certify MEWPs according to the manufacturer’s specifications and recommendations and shall adhere to all applicable standards to ensure safe and reliable equipment operation.
FAR 52.212-1 Addendum
This acquisition will utilize Lowest Price Technically Acceptable procedures; the award will be made to the contractor with the quotation of the lowest price that meets the technical
requirements of the solicitation. Offerors shall price CLINs 0001-0004 and option periods on the solicitation.
The technical requirements are as follows:
1. Offeror shall provide 2-3 past performance references of recent and relevant work documentation performing similar work. Recent and relevant work is identified as work of similar scope/magnitude to this requirement and performed within the last four years. a. Documentation must include a brief description of the services performed, the period of performance dates, at least one POC and their direct contact information from the beneficiary of the services and contract number.
2. Offeror shall provide a maintenance plan and schedule that meets all requirements in the Performance Work Statement.
a. Offeror's service plan shall explain how and when the offeror intends to accomplish the service. The plan should include the types of employees required (labor mix), the proposed schedule of work and address any particular elements of this service that will require special attention.
b. Offeror's maintenance plan and schedule shall not exceed 7 pages (1-sided), the government will only evaluate the first 7 pages.
Only quotations that are responsive to the requirements of the solicitation will be evaluated."
In order to be determined responsive, all quotes must be complete, self-sufficient, and respond directly to the requirements of this solicitation. A complete quote consists of the following: a) Complete the unit price, extended price, and total price blocks along with all information required in the table requesting company information, including vendor signature. b) Fill in required provisions and clauses. If the annual certification has been completed in the System for Award Management (SAM), FAR 52.212-3 does not need to be completed. c) All vendors must have an active registration in SAM at time of quote submission. d) Provide at least two but no more than five references that are recent and relevant for verification. Recent is defined as projects that have been completed in the last three years. Relevance is defined as having the same magnitude of work as the requirement in this solicitation.
