STATEMENT OF WORK (SOW) 1. Background. Well Pump Repair Mitigation to prevent loss of life during an emergency incident. Well provides emergency backup water source for the Madison VA hospital campus. Without the pump, the well cannot operate and we would have no backup emergency water source. 2. Scope. Remove and repair well water pump at the William S. Middleton Memorial Veterans Hospital. Reinstall and chlorinate well per code. Run pump test and collect two safe samples. Provide and install gaskets as required. 3. Specific Tasks. Remove well pump. Repair well pump. Reinstall well pump, providing all necessary parts. Chlorinate well per code. Run pump test and collect two safe samples. 4. Deliverables. Removal, repair and reinstallation will be professionally performed by certified repair personnel with previous commercial experience. Workers will be properly supervised by contractor to ensure quality of work. Materials and work shall be free of material defects. 5. Place of Performance. William S. Middleton VA Memorial Hospital 2500 Overlook Terrace Madison, WI 53705-2254 6. Period of Performance. All work shall be performed during the normal hours of operation (8:00am 5:00pm), Monday through Friday. Work shall commence within 30 days of contract award, and shall be completed NLT 30 September 2024. 7. Performance Monitoring The COR will assure contractor quality through regular monitoring. 8. Security Requirements Vendor will not have access to VA Sensitive Information, patient records, or data. The C&A requirements do not apply, and a Security Accreditation Package is not required. 9. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). Work will take place at the William S. Middleton Memorial Veterans Hospital and campus, a U.S. federal government facility. Engineering will provide relevant schematics to contractor in order to facilitate successful execution of this project. 10. Other Pertinent Information or Special Considerations. a. Inspection and Acceptance Criteria. The COR will certify that the work done under the contract is performed to time and standard, and ensure the inspection and acceptance of products provided incidental to this project. b. Contractor shall minimize the disruption of patient care, staff work and other hospital operations.
