This is a combined synopsis/solicitation for commercial items prepared in accordance with
the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with
additional information included in this notice. This announcement constitutes the only
solicitation; quotes are being requested in response to this notice and a written solicitation
will NOT be issued. This solicitation is being issued as a Request for Quote (RFQ) # 246-
24-Q-0127. Submit only written quotes for this RFQ. This solicitation is a 100% BUY
INDIAN SET ASIDE. This solicitation document and incorporated provisions and clauses
are those in effect through Federal Acquisition Circular 2024-05. The associated NAICS

Welch Allyn Integrated Wall System
1. PURPOSE OF THE PROJECT
The Haskell Indian Health Service (IHS) Clinic has a requirement for the purchase of 14 new integrated
vital sign wall systems for patient care. The Haskell IHS clinic intends to make a single (all or none), firmfixed-
price award for the purchase of the wall systems.
2. DETAILED DESCRIPTION OF THE TECHNICAL REQUIREMENTS
The contractor shall provide the Welch Allyn Connex Integrated Wall System or equivalent that includes
SureTemp Plus Thermometry, SureBP Non-invasive Blood Pressure, BP Cuff and Cord Management
System, Four USB Ports for Accessories, Pulse Rate, MAP, Programmable Alarms, BP Averaging, and
Adult/Pediatric/Neonate Patient Support. The contractor shall provide product meeting specifications with
warranties on all accessories. The integrated walls systems will have the following minimum requirements:

i. SPECIAL REQUIREMENTS
i. All inquiries, comments, or complaints arising from any matter observed, experienced or learned of
as a result of or in connection with the performance of the contract, the resolution of which may
require the dissemination of official information, shall be directed to the government’s designated
representative.
j. DELIVERABLES AND REPORTING REQUIREMENTS
i. Delivery: The contractor shall coordinate the delivery date with the designated government
employee. The government shall provide the contractor access and building access for this sole
purpose. No deliveries can be made after 4:15pm. Driver needs a valid ID.

ii. Contractor Point of Contact: The contractor shall furnish one designated point of contact (POC) to
the government’s designated representative for coordination of supplies and delivery. .
iii. Contractor’s Phone Numbers: The contractor shall provide a toll-free telephone number for service
calls, which must be answered during at least eight working hours, between 8:00 am and 4:30 pm,
Monday through Friday.
k. GOVERNMENT FURNISHED PROPERTY, FACILITIES AND SERVICES
i. The Government shall be available for delivery during normal business hours, and have a pallet jack
available.
l. CONTRACTOR FURNISHED PROPERTY, FACILITIES AND SERVICES
i. The contractor shall furnish all personnel, equipment, delivery service, and lift due to lack of
delivery dock, as required to perform work under the contract.
m. CHANGES TO THE STATEMENT OF WORK (SOW)
Any changes to this SOW shall be authorized and approved only through written correspondence from
the Contracting Officer. Costs incurred by the contractor through the actions of parties other than the
Contracting Officer shall be borne by the contractor.
