NAICS code: 621111 and SBA Sized Standard is $16M PSC Q508 NOTE: All respondents must be registered in the System for Award Management https://sam.gov SCHEDULE OF SERVICES The Contractor shall furnish all key personnel to provide services necessary to perform onsite Hematology Services to eligible beneficiaries of the Department of Veterans Affairs, Robley Rex Medical Center (hereinafter referred to as VAMC), located in Louisville, Kentucky. The Contractor s physician(s) care shall cover the range of Hematology services as would be provided in a state-of-the-art civilian medical treatment facility and the standard of care shall be of a quality, meeting or exceeding currently recognized national standards as established by the American Society of Hematology http://www.hematology.org/Clinicians/Guidelines.aspx. Place of Performance: Services shall be provided on site, VAMC Robley Rex Department of Veterans Affairs (VA) (also known as the Louisville VAMC) located at 800 Zorn Avenue, Louisville, KY 40206 pleting each CLIN for the base year and option years. The Committee/Conference Attendance services CLIN will be billed per agreed daily rate; and therefore, is not billed per CPT code nor needs to be completed below. Throughout the life of the contract the VA will pay the awarded firm fixed percentage of the Medicare rate effective at the time the service is provided by the Contractor. This basis of the contractor s proposed fixed percentage of Medicare rate will be used by the Government for future pricing of additional codes/procedures and for reimbursement under the contract. The basis of estimate used for your fixed price, i.e., the way that your price is calculated, shall remain the same throughout the life of the contract and may be validated by post award audits. The Offeror shall complete the attached price schedule for on-site services at the Robley Rex, VA Medical Center. No CLINS or codes should be left blank. If for some reason a code on the schedule has expired or is for any other reason unusable, offerors shall annotate the Price per Code block with N/A or otherwise notate the replacement code. At the end of each Specialty Area Section, the Offeror must enter a Percent of Medicare that the proposed price represents. The Percent of Medicare entry is extremely important since the contract award will be at that rate. Throughout contract administration, all codes reasonably considered to be included under the specialty areas will be considered in scope. The total extended price figure will be used only to determine the total estimated contract cost for the base and all option periods. The rationale for this approach is that codes and pricing change over time and an award based on Percent of Medicare accounts for those changes. The Contractor s qualified Hematology and Medical Oncology physician(s) shall provide outpatient services, with a minimum of (1) (6) hour clinic session and up to a maximum (3) (6) hour clinic session days per week as needed per VA requirements; each clinic with a maximum of (25) patient slots; one clinic day must be on Friday; focusing on general hematology-oncology including all sub-specialties and inpatient coverage as ad-hoc. Clinic session days will be negotiated and mutually agreed upon between Contractor and the VA Medical Center, to provide coverage for other Hematology/Medical Oncology outpatient clinics during times of staffing shortages or staff absences and valid emergencies as needed. Services will be coordinated through the assigned VA Hematology/Medical Oncology physicians. Services will be rendered at the Robley Rex Veterans Affairs Medical Center (VAMC), 800 Zorn Avenue, Louisville, KY 40206. Contract physicians shall meet or exceed currently recognized national standards as established by The Joint Commission (TJC), and the standards of the American Hospital Association (AHA). In addition, physicians shall comply with all Veterans Health Administration s (VHA) regulations and directives. The Contractor shall provide sufficient personnel who meet all requirements herein and who can be relied on to perform in strict accordance with the contract. The Contractor shall provide physicians who are competent, qualified, and adequately trained to perform assigned duties. The Contractor shall be available to provide outpatient consultation services for the diagnosis and treatment of Hematologic and Medical Oncology diseases. Outpatient services should be offered as needed to meet demand, with a minimum of (3) 6-hour clinic sessions day per week; one clinic day will be on Friday; and a maximum of (3) 6-hour clinic sessions per week as need is identified by VA; each clinic with a maximum of (25) patient slots. It is expected that a provider will be designated for each clinic for the life of the contract to provide continuity of care, when possible, unless circumstances beyond their control necessitate a change. The goal is for all new appointments for outpatient services to be scheduled and seen within 28 days or sooner if clinically indicated. Follow-up clinic appointments are to be seen per established guidelines. Committee/conference attendance is required in this contract, totaling 184 hours each year, to include the committees/conferences list with this contract. The Contractor will provide outpatient consultation services for the diagnosis and treatment of Hematology/Medical Oncology diseases. Contractor staff will work with VA leadership in an advisory manner on tumor board and their composition to improve care for our veterans.
