Could you please clarify why page 3 provides detailed warranty periods for equipment that is not being requested on this RFQ? For example, the RFQ does not include lights, delivery systems, monitor mounts, motors, tubing, or control modules. Answer: Page 3 of the statement of work documents requires a parts and labor warranty. Section 3 specifically states the A-dec standard manufacturer warranties that includes cabinetry, chairs, electric motors. We require an A-dec warranty on all A-dec items listed in the bill of materials. In this case, the cabinetry/furniture warranty requirements are applicable.

Could you please confirm that removal and disposal of existing equipment is to include "equal" items to the products being quoted? For example, since there are no dental lights included in the solicitation, there will not be removal of dental lights. Removal & disposal should be on a "1-to-1 basis, based on new items purchased on this RFQ". Answer: De-installation services will require removal of equal items (older A-dec consoles/cabinetry) and installation of new cabinetry in the existing footprint. There are existing equipment (dental lights, x-rays, etc) that will remain in the dental space. The statement of work requires installation of new A-dec cabinetry to conform to site conditions including existing equipment and utilities connections). Please refer to Section 2.2 and Section 3 of the Statement of Work for more details.

The solicitation includes (2) Dexis Focus IntraOral XRay units, which are not manufactured by A-dec. Could you please update 12.3 accordingly to also require an Authorized Distributor Letter from the manufacturer, which is Dental Imaging Technologies Corporation (DITC)? Answer: Authorized distributor letters are required for all equipment, so A-DEC and Dental Imaging Technologies Corporation (DITC) would both be required.

This requirement is under North American Industrial Classification Standard (NAICS) codes: 339114 Dental Equipment and Supplies Manufacturing; Small Business Size Standard: 750 employees. A tiered evaluation preference will be used to first evaluate quotes from Indian Small Business Economic Enterprises (ISBEE); if no awards can be made to an ISBEE, Indian Economic Enterprises (IEE) will be evaluated. If no awards can be made to an IEE; then, any other quotes will be reviewed.
Definitions. Indian Economic Enterprise (IEE). Any business activity owned by one or more Indians or Indian Tribes that is established for the purpose of profit provided that: the combined Indian or Indian Tribe ownership must constitute not less than 51 percent of the enterprise; the Indians or Indian Tribes must, together, receive at least a majority of the earnings from the contract; and the management and daily business operations of an enterprise must be controlled by one or more individuals who are Indians. This procurement is for NEW Equipment ONLY; no remanufactured or "gray market" items. Vendor shall be an Original Equipment Manufacturer (OEM authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system such that OEM warranty and service are provided and maintained by the OEM. All, warranty and service associated with the equipment shall be in accordance with the OEM terms and conditions. All Equipment must be covered by the manufacturer's warranty. The quote MUST include a copy of the authorized distributor letter from the manufacturer to verify that the vendor is an authorized distributor of the products being quoted.
