This Amendment to the RFP is being issued to provide responses to questions from prospective offerors. Please see the attached Q&A document and epoxy flooring picture attachment.
On October 3, 2024, Amendment 01 was issued.
This Amendment to the RFP is being issued to include the following exhibits/documents on SAM.gov:
a. Exhibit J - Proposal Certifications, Form PUR-466, Rev. 1 (10/14/2021).
b. Exhibit K - Subcontractor Annual Representations & Certifications (SARC), Form SARC (Rev. 3) (04/08/2022).
c. Exhibit L - Construction Environmental, Safety & Health Certification (CESHC).
d. Business Visitor Registration Instructions & Directions
The Scope of Work for this project includes:
- Remove and replace main entrance/exit double doors with frame and all components. (Prime and Paint as well). Doors to be ENERGY STAR certified with a thermal break.
- Remove and replace (2) south side single doors with frame and all components. (Prime and Paint also). Doors to be ENERGY STAR certified with a thermal break.
- Remove and replace (11) EOL windows. Windows to be a minimum double-pane ENERGY STAR certified with a low-e coating.
- Re-Epoxy roughly 500 Sq Ft of concrete flooring.
- Remove and replace EOL commode, sink and mirror in restroom.
- Install a large new sink. (sink onsite already at C0)
- Patch and Paint Walls in the hallway and 2 rooms that were re done.
