Zablocki VA Medical Center (Milwaukee VAMC) located at 5000 W. National Avenue, Milwaukee, WI, 53295, is seeking contractors who can provide two (2) Board Certified / Board Eligible Physicians in Internal Medicine or Family Practice Professional Services for its Community Based Outpatient Clinics (CBOC). A contract is being sought for one year with up to four one-year options that may be exercised at the Government s discretion. The anticipated award date is March 1, 2025. The contractor shall provide one (1) physician who may be scheduled between the Milwaukee VAMC; Union Grove CBOC, 21425 Spring Street, Union Grove, WI 53182; and the Oconomowoc VA Clinic, 1289 Olympia Fields Drive, Oconomowoc, WI 53066. The contractor shall provide one (1) physician who may be scheduled between the Northeast Wisconsin Ambulatory Clinics (NEWAC) locations: Appleton CBOC, 10 Tri-Park Way, Appleton, WI 54914; Cleveland CBOC, 1205 North Avenue, Cleveland, WI 53015; and the Green Bay Health Care Center (HCC), 2851 University Avenue, Green Bay, WI 54311. STATEMENT OF WORK The contractor shall provide two (2) Board Certified / Board Eligible Physicians in Internal Medicine or Family Practice Professional Services on-site in accordance with the specifications contained herein to beneficiaries of the VA. The physicians shall perform full time services between the following locations. The chart below provides the distance between locations. While working at the Milwaukee VAMC, the contractor's employee will be under the general oversight of the Lead Physician of the Primary Care Division. The contractor s employee working at the Union Grove CBOC, and the Oconomowoc VA Clinic will be under the general oversight of the on-site Lead Physician. The VA Lead Physician or designee will provide work assignments and professional direction. The physicians will be granted scope of practice based on his/her expertise and the need of the VA. While working at the Cleveland CBOC and Green Bay HCC, the contractor's employee will be under the general oversight of the Lead Physician at the Green Bay HCC. The contractor s employee working at the Appleton CBOC will be under the general oversight of the on-site Lead Physician. The VA Lead Physician or designee will provide work assignments and professional direction. The physicians will be granted scope of practice based on his/her expertise and the need of the VA. The duties and responsibilities of the Internal Medicine or Family Practice Physicians include, but are not limited to the following: Function as a full team member of the interdisciplinary team serving civilian and military patients at the VA Hospital and Clinics. Provide history, physical examination, diagnosis, and initiation of medical treatment. Initiate and facilitate treatment and transfer in emergency situations. Follow-up office visits for existing medical problems with appropriate diagnostic tests and medication adjustments. Referral of patients as needed to appropriate VA specialists/services or other appropriate health care services. Document assessments and plan of care for each patient in accordance with policies and procedures. Act as a consultant in clinical practice. Collaborate with other disciplines to ensure continuity of patient care. Collaborate and act as a patient advocate. Maintain knowledge of new and current clinical methods. Physician shall be available to accept patient results to be communicated via phone after hours by the lab. The physicians shall be required to work cooperatively with VA Physicians, CRNAs, Advanced Practice Nurses (APN), Nurse Practitioners (NP), Physician Assistants (PA), Registered Nurses (RN), Anesthesia Technicians and a variety of allied health professionals to deliver comprehensive health care services to patients. The physicians shall evaluate and treat approximately 10 patients each 8-hour shift. Medical complexities vary and include cardiovascular, GI, pulmonary, orthopedic, and/or surgical patients. WORK SCHEDULE Physicians will be scheduled 40 hours per week, 8 hours per day, Monday Friday. The shift will be from 8:00 a.m. 4:30 p.m., with a ½ hour unpaid lunch break. No weekends or holidays.
