DuBois, PA Community Based Outpatient Clinic Sources Sought THERE IS NO SOLICITATION AT THIS TIME. This is a Sources Sought Notice only and does not constitute a request for proposals. Submission of any information in response to this notice is purely voluntary. The Government assumes no financial responsibility for any costs incurred associated with this notice. This notice is for market research purposes only. This notice is not a solicitation for offers, nor is it a request for proposals. The purpose of this notice is to identify potential sources and suitable locations and is not intended to pre-qualify or disqualify any potential offers. The Government will not pay for any costs incurred because of this notice. The Government cannot provide any warranty, expressed or implied, as to the accuracy, reliability, or completeness of contents of the furnished information found within this notice. Respondents are advised that the Government assumes no responsibility to award a lease based upon responses to this notice. The issuing of this notice is to conduct market research for sufficient of real property up to 23,062 NUSF/31,134 RSF in the delineated area near DuBois, Pennsylvania for use as a Community Based Outpatient Clinic. The space shall include 215 onsite/outside parking spaces with a minimum of 11 on-site parking spaces with ADA compliant spaces, including two that are van accessible spaces. The anticipated lease will be a full-service lease, to mean all utilities, maintenance, except for janitorial services, are included for a term up to 15 years (firm and soft). The leased space shall preferably be on one floor, the first floor, but not more than two floors (if more than one floor, elevator must be provided and be ADA compliant), in a contiguous space. Delineated Area: The Community Based Outpatient Clinic will remain in the current Congressional District 15 and must be in or abounding the following delineated area: West: Starting in Brockway, follow US 219 south to the intersection with Westerville Rd, turn right onto Westerville Rd and continue head southwest on Westerville Rd, Westerville Rd will turn into Stevenson Hill Rd, continue south. At the intersection of Stevenson Hill and Airport Rd (830) turn right and continue south around DuBois Airport. Airport Road will turn into Washington Twp Hwy, continues south. After going under I-80, Washington Twp Highway will turn into Road 1007 (this road has multiple names but remains 1007-Sensor Rd, Pardus Rd and Sandy Valley Rd.) At the intersection of 1007 (Sandy Valley Rd) and Reynoldsville Falls Creek Rd (950), turn right and continue south to US 322. South: Follow US 322 East until it intersects with Road 153 (State Park Rd) East: Follow Road 153 (State Park Rd) north; in Penfield, Road 153 (State Park Rd) becomes Hoovertown Rd, continue north, then it turns into Penfield Rd continue north to the intersection of road 153 and US 219. North: Follow US 219 West from the intersection of 153 and US 219 to Brockway All businesses are invited to respond to this notice. SDVOSB and VOSB firms must be registered in Veteran Small Business Certification (VetCert) site at https://veterans.certify.sba.gov/#search and have the technical skills and financial capabilities necessary to perform the stated requirements. Only SDVOSB and VOSB firms are requested to submit a Capabilities Statement to VA for review. SDVOSB and VOSB firms may provide a Capabilities Statement if they are interested in participating in this project. Capabilities Statement will include: 1. A summary describing up to three (3) projects of similar size and scope completed in the past five (5) years that demonstrate your experience designing, constructing, and managing Federal leased facilities or health care facilities relevant to the project described above. We encourage all businesses and other interested parties to respond for market research purposes.
