Specifications include, but are not limited to: The State is responsible for administration of the State Plan, including oversight and contract management of all MHCs. The MHCs are available to approximately 44,000 State Plan and Participating Group Members, of which approximately 31,000 reside within the zip code zones close to each MHC location. The State Plan is seeking information from interested parties who could provide State Plan Members, and Participating Group Members, with access to MHCs that focus on integrating their physical and behavioral health needs to maximize health outcomes. The MHCs must have programs that focus on acute and chronic health conditions with documented outcomes shown to improve overall health. Additional services provided in the MHCs include: • Access to 24/7 virtual health; • Access to virtual behavioral health; • Access to health coaching and care coordination; and • Health care integration that collaboratively works within the MHCs and with external providers to coordinate care and provide holistic patient care oversight. A partnership approach between the State and the clinic vendor is essential to achieving these goals. The State seeks information from interested parties who clearly understand these goals and who are capable of meeting the goals in collaboration with the State and Participating Groups. The ideal partner will work closely with the State to effectively operate and manage the MHCs. This will involve communication and coordination with numerous resources within the State and Participating Groups. The ideal partner must also be capable of effectively communicating and coordinating services with multiple third-party administrators.
