Specifications include, but are not limited to: 1. Implement and operate a short-term residential step-down program for a general capacity of 12 individuals with intellectual and developmental disabilities (I/DD) transitioning from hospital/institutional settings to permanent community-based living options. 2. Develop and maintain long-term community-based residential options for individuals transitioning from the step-down program. This endeavor should include a range of options that will offer individuals a choice of the least restrictive setting(s) to meet their wants and needs. This may include maintaining relationships with existing providers, developing relationships with new providers, and/or developing new residential options.
