Specifications include, but are not limited to: Public Health Specialist: 1. Build and maintain connections with community providers including community health centers, health clinics, VDH offices of local health, other wellness organizations and other medical service providers. 2. Provide individual consultation to participants seeking guidance related to communicable disease prevention. 3. Provide or coordinate in-person and virtual classes on best practices for maintaining/improving health and preventing communicable diseases using an evidence-based approach. 4. Provide referral assistance, direct support and advocacy for participants facing challenges acquiring services and resources. 5. Facilitate access to COVID-19 and flu vaccinations and boosters. 6. Facilitate access to treatment and resources for individuals who have contracted COVID-19 or other communicable diseases. 7. Submit a report to DAIL by April 30th, 2024, and September 30th, 2024, on the following: a.) Number of classes provided and total number of older adults who participated in classes. b.) Number of referrals to community providers. c.) Number of full-time equivalents (FTEs). d.) Type of public health professional paid for with this funding. e.) The activities the public health professional is engaged in to advance public health, including a narrative of the activities conducted with community providers. f.) A narrative explaining the successes and challenges that occurred during the reporting period.; 1. Provide coaching to participants seeking guidance related to substance use. 3. Provide or coordinate in-person and virtual classes on substance use, substance use addiction and relapse, and substance use prevention, treatment, and recovery using an evidence-based approach. 4. Provide referral assistance, direct support and advocacy for participants facing challenges acquiring services and resources. 5. Facilitate access to substance use treatment and resources. 6. Submit a report to DAIL by April 30th, 2024, and September 30th, 2024, on the following: a.) Number of classes provided and total number of older adults who participated in classes. b.) Number of referrals to community providers. c.) Number of full-time equivalents (FTEs). d.) Type of public health professional paid for with this funding. e.) The activities the public health professional is engaged in to advance public health, including a narrative of the activities conducted with community providers. f.) A narrative explaining the successes and challenges that occurred during the reporting period.
