Specifications include, but are not limited to: The vendor shall conduct a two-year qualitative program evaluation of the Community Health Team Expansion Pilot with a focus on the following activities: 1. Meet regularly with the Blueprint for Health State evaluation representative(s) and participate in any meetings or workgroups required by the State. 2. Submit a detailed qualitative evaluation plan within one month of execution of the contract. 3. Submit evaluation plan progress reports on a quarterly basis thereafter. 4. Conduct surveys, interviews, focus groups, or use other qualitative data collection methods with each participating Health Service Area to gather participant and stakeholder perspectives on the specified questions in the evaluation framework. 5. Gather and analyze qualitative data. 6. Write qualitative evaluation reports, prepare executive summary of findings, and provide recommendations based on the qualitative evaluation findings to be used by the Blueprint for Health. The formative evaluation report is due to the Blueprint for Health by November 1, 2024 respectively, the final program evaluation report is due to the Blueprint for Health no later than August 31, 2025.
