Specifications include, but are not limited to: The Contractor will use Family Supportive Housing funds to support the staffing costs of one full-time Family Supportive Housing Service Coordinator who will provide case management and service coordination for participant families as they transition from homelessness and ongoing, as needed, to sustain housing stability. The Service Coordinator will provide: customized on-site case management; service coordination; financial empowerment coaching; life skills; tenant education; parent and child resiliency support; and support of addiction recovery. The Service Coordinator will align and coordinate these services with existing AHS programs and initiatives such as Creative Workforce Solutions, Reach-Up, Family Services, ADAP, Children’s Integrated Services, the local Continuum of Care, local recovery centers, and the local Housing Review Team. The Contractor will take a holistic, two-generation approach when providing Family Supportive Housing services. The Contractor will follow requirements outlined in this contract and the FSH Provider Manual.
