Specifications include, but are not limited to: Southern Wisconsin Center, Union Grove, WI 53182, a State of Wisconsin, Department of Health Services (DHS), Division of Care and Treatment Services (DCTS) Facility for the Intellectually Disabled (herein referred to as SWC), is requesting bids for Stockings/Braces Supplies and Services; 1.1.1 Contractor to provide durable medical equipment (compression stockings and braces) and measuring services eligible individuals at Southern Wisconsin Center (SWC); and 1.1.2 SWC desires to arrange for high quality covered supplies that are medically necessary to its to be purchased from Contractor by the State of Wisconsin/SWC. It is understood and agreed to by and between Contractor and SWC that the scope of the agreement shall include: 1.2.1 Contractor shall provide eligible individuals with durable medical equipment, such as compression stockings, braces and measurement services and any additional product classes which are deemed medically necessary and are consistent with Centers for Medicare & Medicaid Services (CMS) medical policy. 1.2.2 Contact the person designated by the Facility on the established order date to review individual’s information and other quantities. 1.2.3 Provide brand preference products to the fullest extent possible unless a substitution product of equal quality is approved by the Facility to avoid shipment delay.
