Specifications include, but are not limited to: 5.1 An occupational health clinic, under the direction of a licensed physician, shall provide the following: 5.2 An annual medical exam performed by a MD, PA, or NP. 5.3 Titmus Vision w/evaluation 5.4 Audio/Audiogram 5.5 EKG with interpretation 5.6 CBC w/differential 5.7 Urinalysis Dip/Complete 5.8 Comprehensive Chem Panel 5.9 PSA (gender specific) 5.10 PAP Smear w/Cytology (gender specific) 5.11 Hemoccult 5.12 TB Skin Test 5.13 Additional tests & labs as needed 5.13.1 Chest X-Ray 1 view (40+ every 5 years or per MD recommendation) 5.13.2 Chest X-Ray (TB) 2 views. 5.13.3 Submaximal Cardiovascular Fitness Test. 40yo then 3-5 years. Annual after 50yo These services may be bundled for annual exams. Any service that may have a recommended interval based on age should be quoted individually. 5.14 Medical practitioner(s) selected shall be licensed in the state of Wisconsin, have competitively priced services and provide generally accepted medical services. 5.15 These services should be under the supervision of a medical group, or physician(s), who are licensed to provide medical services within the state of Wisconsin. 5.16 Scheduling must remain flexible to accommodate employee shift schedules and Owner preferences. 5.17 All exams, tests, and imaging shall be conducted within the City Limits of Janesville as all of the patients will be “on duty” and assigned to a fire or ems response unit.
