Please note: This is a Sole Source Notification. Specifications include, but are not limited to: Dakota Center for Independent Living has the capacity, MFP training, MFP reporting expertise, and the ability to respond quadrant wide quickly to complete the required MFP Grant transition coordination, nursing facility outreach, MFP purchasing, and coordination of services as required by the Centers for Medicare and Medicaid Services (CMS) and the MFP Grant. Dakota Center for Independent Living has the expertise to service all population groups served by the MFP grant, including older adults and persons with physical disabilities. Dakota Center for Independent Living also has capacity and training as it relates to the ADRL Transition and Diversion Program. Referrals are unpredictable, and Dakota Center for Independent Living can respond anywhere in their quadrant of the state at short notice to complete the MFP and ADRL transition coordination services in the manner required.
