Specifications include, but are not limited to: The State intends to make a noncompetitive purchase for the provision of Money Follows the Person (MFP) transition coordination, transition assistance, MFP outreach services, and overall coordination of transition services for adults age 65 years and older and persons with a physical disability in the Northeast quadrant of North Dakota, including those counties serviced by both the Lake Region and Northeast Human Service Centers. North Dakota MFP Nursing Facility Transition Coordination Services are intended to assist MFP qualified consumers with transition from nursing facilities (NF) or hospital settings back into the community. These services are provided through outreach to potential MFP consumers/family members, nursing facility staff, and community agencies, as well as through coordination of transition activities leading to a return to the community. Provision would also include transition and diversion Coordination services through the Aging and Disability Resource Link (ADRL) Flexible Transition and Diversion Program.
