Trauma-responsive setting and treatment approaches 2.1.3.1.3. Family involvement in treatment as clinically indicated 2.1.3.1.4. Discharge planning in collaboration with family, relevant community partners, and State partners 2.1.3.1.5. Attendance at weekly treatment team meetings 2.1.3.1.6. Review of treatment plans, Order of Non-Hospitalization documents 2.1.3.1.7. Coordination of care with outside providers as needed 2.1.3.1.8. May need to attend court hearings as needed 2.1.3.1.9. Collaboration as a team member working in the context of an innovative, comprehensive, holistic, person-centered care model. Overview of level of care/facility role: 2.1.3.2.1. Secure Residential care for individuals on involuntary status 2.1.3.2.2. Maintain compliance with all state and federal laws and regulations for licensed facilities. 2.1.3.3. Hours: 20 to 24 hours per week; 10 to 12 of those hours must be in person each week. 2.1.3.4. There are no “On Call” requirements for this position.
